What should I expect during my exam appointment?
Patients who come to Doctors Collective for a medical cannabis
recommendation can expect to have their blood pressure and pulse
taken. Our Doctors will review your medical history, perform a
targeted physical exam, and answer any questions you may have.
How do I make an appointment?
Call 949.555.1212 to reach the Doctors Collective or setup your own
appointment online. Download, fill
out and bring our online patient history forms with you to your
appointment to save time or all our forms are available in our office
as well.
Can anyone make an appointment for a consultation?
Yes, provided you are at least 21 years old and feel as though
medical cannabis may help with a medical condition.
Do I need to bring any medical records to my appointment?
Patients need to provide medical records listing prior diagnosis
and/or treatment. Examples may include: recent medical records,
prescriptions, physician’s letter, chiropractor’s letter, x-rays, MRI’s,
or any other documentation that you think can help. We also require
a valid form of identification with your picture, such as your California Driver's License, California State Identification Card or Passport.
What if I don’t have any medical records to bring?
Our Doctors will go over your condition with you and may give you
a temporary letter of recommendation. You will be required to
provide medical records via email, fax or at your next appointment
to continue your recommendation for medical cannabis (you will
be able to purchase medical cannabis with your temporary letter
of recommendation).
